Our Support at Home services
Our Support at Home services are provided to help older people remain living at home for longer. The goal is for reablement-focused support. Care is person-centred and goal based, rather than task based.

Our services plays a central role in:
- Falls prevention
- Strength, balance and mobility maintenance
- Cognitive and Communication support
- Nutrition, swallowing and chronic disease management
- Home environment assessment and modification
Our focus is on functional independence, quality outcomes and system efficiency.
Restorative care
Restorative care is a short-term goal focused period of support designed to help older people regain or maintain independence in their daily life at home. Allied Health input is sought with the overall goal of reducing future reliance on ongoing personal care. This input is typically time limited (over 16 weeks generally) and is intended to support people who could benefit from short term intensive support after a health setback.
The aim is to help people stay in their homes longer, avoid more intensive care and improve functional abilities such as strength, balance, mobility or confidence in daily activities.

We provide the following services
- Initial Needs Assessment with an OT
- Assistive Technology or Home Modification Assessment with an OT
- Complex Assistive Technology or Home Modifications Assessment with an OT
- Ongoing ad hoc services as required

- Basic shower chair or stool
- Cutlery
- Meal preparation
- Non slip mats
- Basic wheelchair
- Grab rail
- Simple ramp
- Electric Riser Recliner chair
- Complex wheelchair
- Hoist
- Bathroom modifications
